Jon Harris PA-C Jon Harris Oversees the Mobile Primary care division at Complete Care Medicine. He is a long time native of Arizona and he and his wife have raised their 6 kids here. Jonathan is a graduate of NAU with a Bachelors in Public management and a Masters in Physician assistant studies from University of St. Francis in Albuquerque N.M. Jonathan has developed a network of mobile providers. He is able to coordinate the care of his patients in their own home and can bring in caregivers, home health nurses, physical therapists, mobile X-ray, Lab and social workers. This can all be done within the comfort of the patients home or place of residence.
Our goal at Complete Care Medicine Mobile Care is to provide the highest quality primary care to patients who are home bound or home limited. We are providing this much needed service for the elderly and for chronically ill patients in the comfort of their home or residence.
The Mobile Primary Care model includes a team approach to health care by employing physicians, physician assistants, and nurse practitioners. This team works closely with the patient and their family to provide the most efficient, comprehensive, and holistic approach to their care. We are able to coordinate the care of our patients in their own home and can bring in caregivers, home health nurses, physical therapists, mobile X-ray, Lab and social workers.

Most services which are performed in a doctor’s office could be provided in the patient’s home. These services include:

Do I qualify for an in-home visit?
If one or more of the following apply to you, it is likely that you will qualify for in home visits.
- Someone has to accompany you to the Dr’s office
- Difficult for you to leave the home on your own
- Assistive device is used for walking (e.g. walker or cane)
- You become short of breath with exertion
- It is difficult for you to walk
- In home oxygen used
- You need special transportation.
- You have difficulty with activities of daily living.
- If you leave the home, it takes you days to recover.
- There have been multiple ER visits or hospitalizations
- Cognitive or behavioral impairment
- COPD, Congestive Heart Failure, Pain, or Dementia causing one of the above conditions.
How soon can I be seen by a House Call provider?
We can generally see our patients within 48 hours. Sometimes we are able to do same day appointments.
How often can I be seen in my home?
The frequency of visits all depends on each individual patient. As a rule, frequency of visits are similar to the frequency you would be seen in an office setting.
Can I continue to see my current doctors?
Most insurances require Complete Care Medicine to become the primary care provider. We will then manage referrals and coordinate with specialists.
